New BEAR Form 472 Now Available Online

In July, USAC removed the Form 472 BEAR online functionality as they updated it to reflect some small, required elements. The new BEAR is now available online!
There are two major changes to the form:
1) There is a new column in Block 2 to indicate the discount rate; and
2) The applicant now must include on Block 4 the BEAR check remittance information so vendors know exactly where to mail the BEAR reimbursement checks. (Vendors: this also means that you must use this information to mail the check.)
For applicants, the online BEAR is available at https://www2.sl.universalservice.org/bear/login.aspx. You will need your BEN, PIN, e-mail address, and last name in order to access the online system.
For service providers, the authorized person should log in as usual to the https://forms.universalservice.org/usaclogin/login.asp with the User ID and password, clicks the "472 Online Bear" link on the left-hand side of the page under the Schools and Libraries heading to locate pending BEAR Forms, and then approves and certifies them as appropriate.
