Final Deadline to Submit Item 21s and 471 Certs is May 15

This week USAC will be mailing two sets of letters -- one to applicants that filed their 471 online on or before the March 14 deadline but did not complete the certification process, and the other to applicants that did not submit Item 21 attachments. The final deadline to submit both the 471 certifications and the Item 21 attachments is Wednesday, May 15. USAC will deny FRNs that don't have certified 471s or Item 21 attachments submitted by that date. So if you receive one of these two letters -- even if you're sure you submitted your 471 certification and/or Item 21 attachment -- submit them again. For whatever reason, USAC has no record of receiving them.
• To CERTIFY the 471 online, go to the Apply Online page (http://www.sl.universalservice.org/menu.asp) click the 'Certify Complete' button in the Form 471 column. You must have your Form 471 #, Billed Entity Number, and 471 Security Code. If you don't have your 471 Security Code, or an E-rate PIN, you can print out a copy of the certification pages (pages 6, 7, and 8 of the paper FCC Form 471 at http://www.usac.org/_res/documents/sl/pdf/forms/471_fy05.pdf); provide the same information you provided online; and sign, date and mail to USAC.
• To submit ITEM 21 ATTACHMENTS, use the SLD's Online Item 21 system (http://www.sl.universalservice.org/menu.asp then "Item 21 Attachment"). The best way to submit Item 21 attachments for Priority 2 requests is either by fax (973) 599-6511 or e-mail attachments@sl.universalservice.org.
