Final Week to Post Year 9 (2006-07) Form 470
~ 39 PA Districts Haven't Filed, Listed Below ~

There are still 39 districts that have not yet filed their Year 9 Form 470. If your district's name is listed below, please file the Form 470 in the next few days. If your district's name is not on the list, but you know someone who works in one of the listed districts, please call and encourage them to submit their form. To post a Form 470, go to: http://www.sl.universalservice.org/menu.asp and click on 'Create Form 470.' I encourage you to not use the Interview format, as many technical problems have been reported with this version of the form.
There is still a chance that a district's name is listed but need not be, because a district may have existing contracts for all E-rate eligible services from previous years (and 470's was posted in previous years before those contracts were signed). If this the case with your district, please contact me so I can take you off this list.
The final deadline to post a Year 9 Form 470 is Thursday, January 19. PLEASE DO NOT WAIT UNTIL THIS DATE TO FILE YOUR FORM 470! It will leave you with one day to review your bids, select your providers, sign contracts (with both the district and vendor's signatures), complete your 471 and hit submit.
